Responsibilities:
- Provide comprehensive nursing care to hospice patients in their homes across the New York boroughs.
- Conduct thorough assessments of patients' physical, emotional, and psychosocial needs.
- Develop and implement individualized care plans in collaboration with an interdisciplinary team.
- Administer medications, treatments, and other interventions as prescribed.
- Educate patients, families, and caregivers on disease processes, symptom management, and end-of-life care.
- Ensure accurate and timely documentation of patient assessments, care plans, interventions, and outcomes.
- Coordinate care with physicians, social workers, chaplains, and other healthcare professionals to provide holistic care.
- Valid Registered Nurse (RN) license in the state of New York.
- Minimum of 1 year of clinical experience in hospice, palliative care, or home health nursing.
- Excellent communication and interpersonal skills.
- Ability to work independently and manage time effectively.
- Compassionate and empathetic approach to patient care.
- Willingness to travel throughout the New York boroughs to visit patients in their homes.
